Enter a Report Title and a Description (at least a short phrase is required.) Then, scroll down and, on left side of the form, select the appropriate "Category" for your report. On the right side of the form, "Location Name" field is required. On the map, locate the site you're reporting and use one of the drawing tools to create it. Upload pictures, videos or other links in the fields provided. SUBMIT your report. (Note: once it is approved it will appear on the map and under the Category you selected.)
